Our Policies: The policies listed below are designed to protect all parties. If you would like to enroll your child, you may print this policy form, sign it, and return it to us.

Parent-Participation:

As a parent participation school, I understand that the school requires families to work with them in order to make this school a success. Participation is minimal, and if I can not help, my child's grandparent, uncle, aunt, adult sibling, etc. can help in my place. Just a couple of hours each month is required of our family. Family members will meet with the staff and learn the safety rules, and agreements prior to starting with the school. I/We agree to attend  We will  call Theresa for more information. 298-4278.

Safety & Insurance:
The safety of your child is very important to us. The Sabatino Memorial Family Resource Center strives to conduct its programs safely & expects all participants to follow safety rules & instructions which have been designed to protect and enhance the participants’ safety. The SMFRC, carries general liability insurance for its operations. There is a zero tolerance for physical/verbal abuse of our children, staff, or fellow parents. Under no circumstances will “hitting”, spanking, or screaming at your child, be allowed at the school.

Release from Liability: I recognize and accept that there is a risk inherent in participation in group and/or physical activity, including the classes offered by The Sabatino Memorial Family Resource Center. I agree to assume the full risk of any physical injuries, damage, or loss regardless of the severity, which I or my child may sustain as a result of participating in any and all activities connected with or associated with any SMFRC  program. I, on behalf of myself, my spouse, my child/children, nanny, friend, and any caregiver or other person attending with my child, hereby and forever waive, relinquish, fully release and discharge The Sabatino Memorial Family Resource Center,  its directors, instructors & staff, and the landlords, agents, & employees of the facilities in which classes are held, from any and all liability or claims resulting from bodily/personal injury, property damage, medical expenses or other loss sustained in connection with such classes. I understand that my child/children will be playing/learning in The Guadalupe River Park, and will be supervised carefully at all times. The home is across from the river. There is also a small pond in the park.

Permission to Secure Treatment: In the event of an emergency, I authorize The Sabatino Memorial Family Resource Center, to secure from any licensed hospital, physician, or medical personnel any treatment deemed necessary for my child’s immediate care & I, the parent/guardian assume responsibility for payment of all medical services rendered.

Photo Permission: All participants agree that any photography or video taken while participating in class or special events, may be used for promotional purposes for The Sabatino Memorial Family Resource Center. If you do not agree, please add a notation and signature to the bottom of this page.

Non-Discrimination Policy:

We do not discriminate. Children of all ethnic backgrounds are very welcome to attend!

Our “Rights”: We reserve the right to admit families to our program who we feel are a “fit” with our philosophy of care. We also have the right to dis-enroll a child under circumstances where we feel we can not keep the child safe (a child who does not listen). We will also dis-enroll children who have repeated incidents of negative behavior that impacts the other children in a negative way. We will do our best to help all children and families-in the event that we can not, we will be forced to dis-enroll a child. Please understand we have to consider the group as a whole.

I have read and & fully understand paragraphs 1-7 above. I understand that due to the small size of  classes, that all fees are non-refundable. Families pay a monthly fee/donation which will help cover the costs of maintaining the facility, the outside activities brought in such as musicians, artists, cooking teachers, etc. Donations/fees will also cover food, supplies, and other items needed to run the Italian school. I understand the the staff and faculty are completely volunteer.

The school is a 501c 3 non-profit, and we are willing to pay a monthly fee/donation to make this work. No refunds for days missed. No vacation days. A list of holidays is posted on the parent board, and the monthly fee/donation is due, regardless of holidays/days missed.
